This Membership Monthly Fee Payment below is a piece of paper created using Microsoft Word that you can use as a proof of recurring membership fees that are being paid by all active members in your club. You can use it for example, fitness club, dance courses, sports club etc.

To use this template, put your organization name and payment year. Then start to fill the number, member code, member name, joint date, and their monthly fee. Everytime your member pay the fee, you can write it in related month boxes. You can check it or put the payment date. If they pay in advance for several months, you can type the same dates in those months.

Remember to put this piece of paper in a good place to prevent any disputes that may arise when you lose this proof payment sheet.
