This, however, is the basic function of a document. Any hospital/clinic has its own method of recording. Usually, the patients have to queue before getting any service. There will be staffs who will record your birth, name, address to access your previous visit. Just print your queue ticket, waiting for the call, and when you are called, all you have to do just mention your medical ID or your birth date and then you can register for medical services.
