There are 3 until 7 column template that you can select and change its title based on your needed visitor information. And you can print it with your company letterhead.
These are short description of titles that you might need or change from those templates.
- Name, visitor name
- Company, visitor company
- ID, ID that visitor leave at receptionist
- Person to meet, person that visitor want to visit
- Purpose, usually meeting is the most common purpose
- Visit Time, time that visitor arrive
- Sign, visitor signature
