Perhaps, you already have experiences on writing your name and sign it in a piece of paper at security or reception desk, when you are entering a building to visit your colleagues company before they let you pass trough. And this policy is different in different building. For example, instead of just writing your identity,  building security people will ask you to leave your identity card, or ask you to be finger or photo printed before allowing you to pass them. Eventhough the process is not the same, the aim is the same. It is for security purposes.

These building visitor sign in sheets will give you ideas about needed information from building visitor before security staff allow them to enter the building. There four until seven column templates that you can choose and modify based on your building policy. Feel free to change the title at the top of every columns.
